"I retained A. Ali Yusaf at this location around May 2016 for a construction scaffold work accident I suffered on May 04, 2016 which resulted in traumatic injuries to my body. This same firm also uses the name YUSAF & ASSOCIATES. The day I signed my retainer at this location A. Ali Yusaf along with Stephen Skor was present. The only retainer I signed was for A. Ali Yusaf at his Richmond Hill Queens location every time I asked about my case both of them would say it takes years to get your case in front of a judge. They took copies of all my taxes from 2013, 2014, 2015, and in 2017 they asked for my 2016 taxes from January to May 04, 2016 the day of my work accident/injuries.They copied my social security number, birth certificate, resident card, my wife and two daughters social security numbers, my two daughters birth certificates, my wife state ID and my marriage license, they said my wife and daughters are included in my civil case. Around fall 2016 they told me my case was filed in Supreme Court Queens County, then their paralegal around August,September told me that Mr.Stephen Skor went to Supreme Court Kings County to pick out jury for my case, my wife and I was shocked because they never told us about that. Please note that I never signed a retainer for Stephen Skor. I did two depositions the first was a deposition by a grinder expert in 2016, then I was deposed about how my work accident happened on May 04, 2016 in 2017. In 2018 I was deposed again for the emergency spinal surgery I had in November 2016, hand surgery, therapies, treatments. None of the defendant attorneys denied my work accident and injuries. I noticed if I did not find out about my case they was not informing me or giving me updates every time I asked they said they was waiting on a date with a judge that was in June 2018 they also told me that if I want I am free to find another attorney. So my wife and I decided to find another firm/attorney for my civil case Index No: 157714/2016, 157714/16. I found out my index number when I retained another attorney in July 2018. Mr. Stephen Skor took me in June 01, 2016 to meet with some attorney to start my workers compensation claim WCB Case No: G1652616 that firm was located in Kew Gardens Queens New York, I signed one retainer on June 01, 2016 for that firm to handle my workers compensation claim. Since I migrated to the U. S. A in November 2021 I have only lived in Queens County and the only state I have lived in is New York. Time went by I became pro se on June 18, 2020 and is still pro se in all my cases. I received my WCB records that is where I found out that my signature was forged several times while A. Ali Yusaf and his associate Stephen Skor had my civil case they forged my signatures and released my medical and workers compensation records to the defendant attorneys in my civil case without my consent and authorization. I also found the summons that A. Ali Yusaf filed in my civil case in workers compensation records, along with all my tax records they requested from me all was filed in my workers compensation records WCB Case No: G1652616 this was all done without my consent and authorization. Then in July 05 2017 some attorney I never met, never retained or spoke to filed for social security disability benefits using my tax records, I was hospitalized and recovering from a PLIF surgery I had on July 05, 2017, I was still hospitalized when this attorney filed for SSDI benefits then a few days later that same attorney faxed from the office located in 25 Newbridge Road, Suite 203, Hicksville New York 11801 to the Jamaica Queens Social Security Office a fraudulent fee agreement with my signature forged requesting all my Social Security Records, SSI records. The workers compensation firm that Mr. Stephen Skor took me to someone forged my signature several times in that office and different files was created in my WCB Case No: G1652616. I really don’t understand if Mr. A Ali Yusaf has something personal against me because of my religion that is why he and his associate committed fraud."
